Issues with dynamic call transfers and extensions

We currently use dynamic transfers and use extensions, see below.

Recently we’ve been having issues, getting this message. It doesn’t happen every time, only about 30% of time. Is it because we are using extensions? Or because something in the formatting is incorrect?

Can you enable “keep it raw.” If this doesn’t work, change the LLM for this node to GPT-4.1, as sometimes the LLM can hallucinate and fail to return a valid phone number.
